Many dogs benefit greatly when combining the added support and mobility of the brace with physical therapy.  Check out the lists of rehabilitation facilities in your area and see how they may be able to help your dog.  You may also want to reach out to your local vet for recommendations.

The two accredited canine rehab programs are Canine Certificate Rehabilitation Program (CCRP) and Certified Canine Rehabilitation Therapist (CCRT).
